Ultherapy PRIME®: When It May Be Appropriate—and When Another Approach May Be Better

Some patients considering Ultherapy PRIME® have facial volume loss in addition to, or instead of, skin laxity. Although these changes can look similar, they have different underlying causes and may require different treatment approaches. Focused ultrasound is designed to deliver controlled thermal energy to selected tissue depths and induce a gradual tissue-remodeling response. It does not replace lost facial volume. Before choosing a device or treatment, it is important to first understand what has actually changed in the face.

Sagging and Volume Loss Are Different Issues

Sagging, or skin laxity, refers to downward changes in the skin and supporting tissues as elasticity and structural support gradually decrease. Multiple age-related changes may contribute to this process, including changes in dermal collagen and elastic fibers, as well as the deeper supporting structures of the face.

Volume loss is different. It occurs when the volume of facial tissue itself decreases.

Facial fat is not distributed as a single uniform layer. It is divided into several compartments, and these compartments can change at different rates with age. When areas such as the temples or anterior cheeks lose volume, the overlying skin may appear looser or more deflated. In these cases, the primary issue may be volume loss rather than laxity alone.

Changes in facial appearance when lying down or tilting the head back may sometimes provide a clue, but posture alone cannot reliably distinguish between laxity and volume loss. In many patients, the two occur together.

What matters clinically is determining how much each factor is contributing to the current appearance.

Which Tissue Layers Does Ultherapy PRIME® Target?

Focused ultrasound works by concentrating ultrasound energy at specific tissue depths to create controlled thermal coagulation points.

A simple way to understand the principle is to think of sunlight being focused through a magnifying glass. Rather than heating the entire path through which the energy travels, the energy is concentrated primarily at the focal point.

This allows thermal energy to be delivered to deeper tissue layers without surgically incising the skin.

At these focal points, microscopic areas of thermal coagulation are created, followed by a gradual tissue-healing and remodeling response. Collagen remodeling and new collagen formation are part of this process.

Two points are particularly important to understand.

The first is when to assess the results.

Changes should not be judged solely by how the face looks immediately after treatment. Tissue remodeling takes time, and results may continue to develop over the following weeks and months. For this reason, the day after treatment is generally too early to determine the final outcome.

The second is individual variation.

Tissue response varies from person to person, which means that the degree and timing of visible change can also vary even when the same type of treatment is performed.

Why Is It Important to Check the Approved Indications?

Ultrasound lifting devices are regulated medical devices, and their approved uses are defined by specific anatomical areas and intended purposes rather than by broad phrases such as “the entire face.”

In Korea, the approved uses of Ultherapy PRIME® include lifting of the brow, sagging jawline, and upper chest area, as well as improvement of wrinkles in the décolletage area.

For patients, this distinction is important.

Marketing expressions such as “full-face lifting” should not be confused with the regulatory indications for which a medical device has been approved. When considering what a treatment may realistically achieve, the approved indications and an individual medical assessment provide a more appropriate starting point than advertising slogans.

When May Ultherapy PRIME® Be Considered?

Ultherapy PRIME® may be considered when laxity and loss of tissue support are among the primary concerns and significant hollowing is not the dominant issue.

It may also be considered in patients who are beginning to notice reduced facial definition or early-to-moderate changes in the jawline but are not currently considering surgical lifting.

For patients who prefer a non-incisional approach and cannot accommodate a prolonged recovery period, focused ultrasound may also be one of the treatment options discussed during consultation.

Ultimately, suitability depends on whether the patient's skin, soft tissue, facial structure, and treatment goals are appropriate for the procedure.

When Might Another Approach Be More Appropriate?

When volume loss is the primary concern

Focused ultrasound does not replace lost volume. If hollowing is a major component of the facial change, ultrasound lifting alone may not provide sufficient improvement.

In patients who already have significant volume loss, excessive tightening without considering volume may also make the face appear thinner or more tired. In these cases, volume loss and laxity should be assessed separately before determining the treatment plan.

When there is very little subcutaneous fat

Patients with relatively thin soft tissue require more careful assessment of treatment depth, energy delivery, and anatomical structure.

A thinner face does not automatically mean that focused ultrasound cannot be performed, but treatment planning should be particularly individualized.

When there is substantial excess skin

If skin laxity is advanced and there is a significant amount of excess skin, a non-incisional energy-based treatment may have limitations. In some cases, a surgical approach may provide a more appropriate option.

When there is an active skin condition or another medical concern

Treatment may need to be postponed or reconsidered if there is an active infection, inflammatory skin lesion, or another medical condition affecting the treatment area.

Individual medical history and current health status should therefore be reviewed before treatment.

Limitations and Risks to Know

Discomfort can occur during treatment.

Patients may experience a deep aching, tingling, or tender sensation when energy is delivered to deeper tissue layers. Areas closer to bone may feel more sensitive, and the intensity of discomfort varies considerably from person to person and by treatment area.

Temporary redness and swelling may occur after treatment and generally improve over time.

Less common adverse effects may include temporary sensory changes or temporary weakness related to nerve irritation. Localized fat atrophy is another potential adverse event that should be considered when treatment depth and energy are planned.

Any persistent, worsening, or unusual symptoms after treatment should be medically assessed.

It is also important to have realistic expectations regarding duration.

Changes associated with tissue remodeling are not permanent, and the aging process continues after treatment. How long the visible effect lasts varies depending on factors such as age, skin condition, facial structure, lifestyle, and individual biological response.

When can I see changes after the procedure?

Since it takes time for tissue remodeling to progress, judgment is made by looking at the progress over weeks to months, not immediately after the procedure. Conversely, it is also too early to judge the effect just because there is no change the next day.

Does it hurt?

Pain may accompany the procedure. A dull or aching sensation is often felt when energy is delivered to the deep layers, and areas closer to the bone may be more sensitive. If pain control is necessary, it can be performed under anesthesia after prior consultation with anesthesiology staff, but this requires prior reservation and is decided by considering the risks of anesthesia itself.

Is one session enough?

It is decided by looking at the initial condition and the reaction after the first treatment. Are there any special precautions after the procedure?

On the day of the procedure, just stick to washing your face and basic moisturizing, and do not rub the treated area hard. For a few days, it is better to avoid environments where heat is applied for a long time, such as saunas and steam rooms, and excessive drinking.
